There is something related to battery pack in the UPS Log that I am using
UPS recently received a replacement due to a malfunction, and firmware is up to date
What's the cause?

Unless a the firmware was already upgraded after you installed the replacement UPS, I recommend upgrading the UPS firmware, ensuring all battery packs are connected to the UPS before and during the upgrade. If you have already performed this, please reach out to your local APC Tech Support for further troubleshooting.
